Die Behandlung der Frtihgeburt hat in der letzten Dekade einen entscheidenden Fortschritt gemacht. Butterworths, London). Interessant ist es, daE sich die zaW der Frtihgeborenen tiber die Jahre kaum verandert hat. Vom Einsatz der Tokolytika versprach man sich, die Inzidenz der Frtihgeburtlichkeit zu senken. DaE dies offensichtlich nicht gelungen ist, lag an mehreren Umstiinden. Ins Gewicht fielen dabei die manchmal gravierenden Nebenwirkungen der -Mimetika, z.B. die allergische Dermatitis (Horawith JJ, Creasy RK (1978) Am J Obstet Gynecol 131: 225), sowie das Lungenodem (Wolff F, Meier U, Bolte A (1979) Z Geburtshilfe Perinatol183: 343). Die Pathomechanismen, die zu Nebenwirkungen ftihren, sind so untibersichtlich und mannigfaltig, daE man versucht, neue -Mi- metika mit selektiver 2-Wirkung zu entwickeln bzw. durch eine Zusatztherapie unerwtinschte l-Wirkung zu neutralisieren. 1970 wurde erstmalig das Hexoprenalin als ein neues -Mimetikum in der Behandlung des Bronchospasmus vorgestellt (Deutsch E, Irsigler K, Kraupp 0 (1970) Hexoprenalin. Springer, Berlin Heidel- berg New York). Hexoprenalin, ein verdoppeltes Isoprenalin, hatte nach den Ergebnissen der damaligen Untersuchungen v.a. eine 2- Wirkung. Es dauert lange, bis Hexoprenalin auch zur Hemmung der vorzeitigen Wehentiitigkeit eingesetzt wurde, obwohl die I.
