Surgery of the central nervous system has increasingly stepped out of the narrow boundaries that were imposed on it just a few years ago. Von Bergmann's well-known statement that brain surgery is surgery of the central convolutions no longer exists justifiably. Almost all areas of the contents of the skull are now surgically accessible. The purpose of this thesis is to determine the anatomical basis for the pathways to the individual regions of the central nervous system, to critically examine the various methods used and to pave new paths on the basis of our anatomical knowledge. The cooperation of anatomists and surgeons was desirable. The pathway that has been found to be anatomically correct should also be checked by the clinician for its suitability. Here the experience which one of us was able to gain as a long-time assistant in the field of surgery of the central nervous system on the rich material of the Clinic for Astronomy was of great practical use. In recent years, the experience of war in the case of injuries to the central nervous system has been very useful to us, both anatomically and surgically. It was not our intention to give a complete overview of all the surgical methods indicated and carried out, but rather we thought it more expedient to present those methods which seemed to us to be correct from anatomical reasons and which have proved useful to us in man.
