Indication and practice of cerebroprotective measures in neurosurgery by K Filos
The prevention and treatment of permanent neurological deficits due to acute perioperative oxygen deficiency of central nervous structures has become one of the main tasks of anesthesiological management in recent years, especially in view of the successful further development of cardiopulmonary resuscitation techniques. Neurosurgical interventions are naturally particularly linked to the problems of cerebral ischemic or hypoxic hypoxia, e.g. due to intraoperative tissue trauma, genital surgery, swelling of the skin, air embolism during operations in a sitting position or also accompanying deep controlled hypotension. From this point of view, they offer a special challenge for the caring anesthesiologist, which he seeks to meet both by general measures in the sense of maintaining sufficient local and global cerebral oxygen availability and by specific measures for a possible improvement of cerebral ischemia tolerance. In order to discuss the latest developments in the field of perioperative skin protection, neurosurgeons and anesthesiologists met in Frankfurt for a round table to exchange experiences in this field, which is important for both disciplines. Based on the results of our own clinical studies as well as on more recent aspects of the pathophysiology, pathobiochemistry and pharmacotherapy of cerebral Oz deficiency syndrome, an attempt should be made to develop starting points for new therapeutic guidelines that may in the future be developed into a promising concept for the prevention and treatment of cerebral ischemian injuries. The lectures given by the speakers as well as essential points of the subsequent discussion are the content of this book. The editors would like to take this opportunity to thank Eli Lilly GmbH, Bad Homburg, for their generous support of the event.
