In the case of multi-fragment fractures with isolated cortical fragments, the question arises as to their value as an autograft. In an experimental setup in animal experiments, the formation of new bones in the stable and unstable diaphysaric defect of a long bone was investigated as a function of the transplant inserted. Autologous spongiosa was compared with autologous cortical microspans in different concentrations and in graduated instability of the graft site. Radiological and histological methods could be used to show that autologous, low-density cortical microspans in the stabilized diaphysaric defect have similarly good osteogenetic properties as the low-density autologous spongiosa. Increasing compaction of the graft and growing instability of the graft site led to the deterioration of new bone formation.
