German Society for Anaesthesia and Resuscitation by P Lawin
After describing the indication for intra-aortic balloon pump therapy, the data of 12 patients treated with this method in Hamburg are presented. In helping the patients, the therapy had been effective with regard to the initial cardiac situation. Four patients in this group died of additional organ damage. like shock lung. Bronchopneumonia and urosepsis. The therapy was successful in two patients. They survived. 289 INFLUENCES. THAT DETERMINE THE ISCHAEMIA TOLERANCE OF THE ARTIFICIALLY IMMOBILIZED HEART By Fr. Kalmar. N. Bleese. U. Kirsch. H. Pokar and G. Rodewald In open heart surgery, a number of operations are necessary. to shut down the heart in fear. The ischaemia tolerance time of a healthy normothermic heart lies in the Gr613 arrangement of 60 minutes. In cardiac surgery, however, the hearts to be operated on are pre-damaged in various ways. Thus, it is necessary. intraoperatively, to prolong the ischaemia tolerance of the heart, i.e. to reduce the oxygen requirement or to block the degradation of energy-rich phosphates. The first can be achieved by hypothermia, the second by the use of cardioplegic substances. Experimental and clinical studies have shown that the cardioplegin introduced by KIRSCH is effective in prolonging ischaemia tolerance (4, 5, 6, 7). According to the postulates of BRETTSCHNEI- DER ( ), the following factors are of great importance in ischaemieteOlerance: before ischaemia, the storage of energy reserves and the reduction of energy turnover. during ischaemia small energy expenditure and after ischaemia possibilities of rapid recovery. An experimental examination on the guinea pig heart of IMHOLZ (3) shows, measured at the time of rigor mortis onset.| ISBN 13 | 9783540068037 |
