Blood clotting and wound healing are basic prerequisites for successful surgical activity. A central component of these two reparative services of the human organism is the formation of fibrin. It stabilizes the initial thrombocytic haemostasis, leads to temporary wound closure with the formation of an infection barrier and promotes scarring by stimulating fibroplast growth and subsequent collagen synthesis. For almost 80 years, attempts have been made to use these fibrin effects therapeutically by local application of plasma preparations. However, wider application was only possible when the production of corresponding coagulation factors - especially fibrinogen - was achieved in sufficient quantities, concentrations and purity in the early 1970s. According to initial success reports from Austrian working groups, this new technique has also been tested in German hospitals since 1976. It has now spread to almost all areas of surgical medicine, including small and medium-sized hospitals. In the early years, attitudes towards this so-called fibrin bonding were often emotionally emphasized. Exaggerated expectations and occasionally a lack of experimentation were met with sceptical rejection by the majority of surgeons. In the meantime, however, proven areas of application and indications have emerged in many surgical disciplines, in which fibrin adhesion has made surgical facilitation in some cases, higher postoperative safety, and in some cases also made novel therapeutic procedures possible.| ISBN 13 | 9783540132851 |
