Dangers of intramedullary nailing in shock by Gerald E Wozasek
The discussion about intramedullary nailing has not died out since it was introduced by Gerhard Ktintscher and is still topical and controversial. The questions about biology and biomechanics that have been asked since the beginnings are being answered more and more relevantly with refined research methods. We know that 1. that by drilling through the way of increasing pressure in the medullary space, an intravasation of its contents in the systemic circulation is brought about; 2. that the medullary fat and also medullary particles are intercepted in the pulmonary stream, which temporarily impair the pulmonary circulation and the cardiac function, but do not cause permanent damage under normal conditions; 3. that the intramedullary nailing carried out in the injured person's state of shock constitutes a life-threatening danger to the injured person. W ozasek tries to answer open questions under standardized conditions in animal experiments by investigating the mechanisms of fat infiltration during intramedullary nailing - imitating the state of shock - and by trying to define the effect on lung function. The methodologically sound experimental set-up on the one hand and the comparative consideration of the results of other authors on the other hand allow a remarkable insight into the pathophysiological processes surrounding shock, fat intravasation, fat embolism syndrome and intramedullary osteosynthesis. I consider Wozasek's work to be a remarkable milestone. It sheds some light on the darkness without fully illuminating it, confirms what has gone before, and raises new questions: all in all, highly topical and of noteworthy clinical relevance.| ISBN 13 | 9783540575122 |

"I was paying $11.99/month for Kindle Unlimited but realized I was renting books, not owning them. Bought this PDF and it's mine forever. Never going back to Kindle."
"Switched from Kindle to these PDFs. Same book, but I actually OWN it now. No monthly fees, no app required. Worth every penny."
"Kindle Unlimited kept raising prices. This is a one-time purchase and the file is permanently on my iPad. Best decision I've made for my reading habit."
"I compared Kindle vs this store. Kindle is cheaper monthly but you don't own anything. Here I paid once and the PDF is mine for life. Highly recommend."
"Left Kindle behind after they removed a book I was reading mid-series. With this PDF, nobody can take it away from me. True ownership feels good."
"Was skeptical about paying $18.95 when Kindle shows $3.99, but then I did the math — Kindle Unlimited is $143/year! This is way better value long term."
"Love that I can read this on any device without installing the Kindle app. The PDF works on my laptop, phone, and tablet seamlessly. So much freedom."
"My Kindle subscription was getting expensive. Switched to buying PDFs here and I'm saving money while actually building a permanent digital library I control."
