Central European Congress of Anesthesia? by B Haid
W. F. List A preoperative outpatient clinic is to be understood as an organizational form that enables early preoperative findings to be determined by anesthesiologists to determine the anesthesia ability and the anesthesia risk. Surgical patients with elective procedures should be seen in such an outpatient clinic either before they are admitted to hospital or immediately afterwards. The examinations are carried out in the separate premises of the anaesthesia department or the anaesthesia institute or the anaesthesia clinic. This ensures that the anaesthesiologist comes into contact with the surgical patient at an early stage and can collect all the necessary findings to determine the anaesthesia's ability (suitability for anaesthesia) and the anaesthesia risk. Between 25 and 40% of our patients show medical findings in addition to the surgical disease, which must be taken into account (Kyei Mensah et al. 1974). Surgical risk and surgical lethality are significantly increased by medical comorbidities (Goldman et al. 1977). Until now, it was the case - at least in my institution - that preoperative findings had to be collected by the surgical patient himself in several outpatient clinics and laboratories. In this case, the surgeon very often requested an internist, who usually determines the suitability for surgery with "gentle anesthesia". The anaesthesiologist did not see the patients until the evening before the operation. An often very long list of patients had to be assessed and premedicated in a very short time. It was not uncommon for the anesthesia capacity to be clarified even with inadequate findings. Discontinuation of the operation led to difficulties with the patient, with the surgical program and with the surgeon.| ISBN 13 | 9783540109426 |
