In recent years, the search for a suitable bone substitute has led to experimental, sometimes clinical testing of a wide variety of substances and substance mixtures (composites). Despite intensive research, however, the mechanism of action of the "common" bone substitutes has not yet been sufficiently explained and the behavior of the material after implantation has not been predicted with genetic reliability. Based on the results of research in immunology and the widely discovered mediators of bone cultivation and bone resorption, our predominant, more "morphological" understanding of bone healing may be extended by a more cybernetic representation of the regulation of the cells involved in the formation and breakdown of bone tissue and their specific performance and products. Only after the integration of morphology, biochemistry and immunology can the mechanistic ideas about and the demands on a bone substitute be revised. The aim of this book is to create a catalogue of requirements for the desired performance of bone substitutes, to classify the substances offered as bone substitutes, to describe their postulated mechanisms of action in summary, to critically examine the actual activities of the various individual substances and composites observed during the examination in a standardised animal model - taking into account osteological, biomaterial and immunological results - in order to provide a realistic outlook on the development of bone substitutes in the future.
